Myotonic dystrophy, mitral valve prolapse, and cerebral embolism.

چکیده
CARDIAC abnormalities are the rule in hereditary myopathy, and myotonic dystrophy (MD) is no exception. Conduction disturbances are most common, manifested by first degree atrioventricular block, intraventricular conductive defect, and by evidence of disease in the entire conductive system on His bundle electrogram. The association between mitral valve prolapse (MVP) and MD has been reported as a familial trait. Cook et al have reported a patient with MD, MVP, and cerebral infarction. However, there are grounds for questioning the source of the embolism in their patient. We report a second patient with MD, MVP, and a transient ischemic attack in whom we believe the mitral valve was the source of the embolization.

Left ventricular diastolic function in congenital myotonic dystrophy.
OBJECTIVE Examination of left ventricular function and conduction abnormalities in myotonic dystrophy. DESIGN Twelve patients (median age, 13.7 years) with myotonic dystrophy had detailed electrocardiography and echocardiography performed. Echocardiographic parameters were compared with body surface area (BSA) matched median normal values. RESULTS Fractional shortening was slightly reduced ...
متن کاملRoutine echocardiography in patients with myotonic dystrophy type 1.
BACKGROUND Myotonic dystrophy type 1 (DM1) is an autosomal-dominant disease. One third of DM1 patients die suddenly, most of them due to the heart conduction abnormalities and arrhythmias. The aim of this study was to analyze echocardiographic findings in a large cohort of DM1 patients. METHODS This retrospective study comprised 111 patients and 71 healthy controls (HCs) matched for gender an...
متن کاملCardiac involvement in congenital myotonic dystrophy.
Seven young patients (mean age 19 years 8 months) with congenital myotonic dystrophy and with defined symptoms at birth were investigated by electrocardiography and echocardiography. None had cardiovascular symptoms. Electrocardiograms or echocardiograms or both were abnormal in all patients. Atrioventricular and intraventricular conduction defects were the most common electrocardiographic abno...
متن کاملStructural myocardial involvement in adult patients with type 1 myotonic dystrophy
Myotonic dystrophy type 1 (DM1) is the commonest muscular dystrophy in adults, affecting multiple organs in addition to skeletal muscles. Cardiac conduction system abnormalities are well recognized as an important component of DM1 phenotype; however, primary structural myocardial abnormalities, which may predispose these patients to congestive heart failure, are not as well characterized. We re...
متن کاملMarcus-gunn phenomenon with- mitral valve prolapse
Marcus-gunn phenomenon is a congenital and sometimes hereditary anomaly, in which a ptotic eyelid retracts momentarily when the mouth is open, or the jaw is moved. These associated movements attributed to abnormal connections between the central mechanisms innervating the pterygoid and elevator muscles. Sometimes this anomaly is associated with dual,s retraction syndrome, duplex kindneys and ur...
